Cisto de retenção mucoso no seio maxilar e remanescente dentário como causa de sinusite maxilar: relato de caso.

Abstract

Introduction: The paranasal sinuses are pneumatic structures that constitute the face, among them, the maxillary sinus stands out because it is the largest and in many cases maintain an intimate relationship with the posterior superior dental elements. During surgical procedures in the region, fragments of dental origin or not can move into the sinus accidentally generating complications to the patient, such as the installation of mucous retention cysts and maxillary sinusitis. Treating the patient correctly is of fundamental importance for the resolution of cases, with the Caldwell- Luc pathway performing optimally, with easy execution and satisfactory results. Objective: This study aimed to report the surgical removal of a dental fragment and mucous retention cyst in the maxillary sinus by the Caldwell-Luc technique as a treatment for recurrent maxillary sinusitis. Clinical case report: A 36-year-old female patient was referred to the oral surgery service of the Federal University of Campina Grande, Campus Patos – PB. The patient report pain in the middle third of the face. Extra and intraoral clinical examination did not show any abnormality. Analyzing the panoramic radiograph, a radiopaque image was observed in the region of the element 25, suggestive of dental fragment. As a result of a more assertive surgical planning was made by cone beam computed tomography (CBCT), which facilitated the surgical approach. The procedure was performed, where the dental fragment, mucous retention cyst in the maxillary sinus were removed by means of the Caldwell- Luc technique and tooth extraction 26 due to failure of the endodontic therapy, without intercurrences, and in the postoperative period, the patient has evolved satisfactorily. Conclusion: It can be concluded that the Caldwell-Luc technique for surgical approaches in the maxillary sinus is effective, easy to perform, low cost and satisfactory. Imaging tests, such as CBCT, is a valuable diagnostic exam, as well as surgical planning, due to the high resolution and accuracy of the images obtained.
