Tratamento cirúrgico conservador de extensa lesão cística em mandíbula.

Abstract

Conservative surgical treatment of an extensive cystic lesion in the mandible. Radicular cyst is an inflammatory lesion, usually asymptomatic, and may present with increased volume in the region, mobility and tooth dislocation, facial asymmetry, pathological fractures and damage to adjacent vital structures. Radiographically, a radiolucent image, usually circular or ovoid, with radiopaque margins and varying in diameter is observed. Numerous treatments have already been described in the literature for the management of root cysts, among which we have: endodontic treatment, tooth extraction, enucleation, marsupialization, decompression or association of these modalities. The present article aims to report the occurrence of an extensive periapical cyst in the mandible. A 82-year-old female patient, feoderma, attended the UFCG dentistry clinic, reporting an increase in volume in the anterior region of the mandible. At the intraoral examination, several root remains were observed, swelling in the anterior region of the mandible, and palpation of the patient in the region. For a better delimitation of the lesion, relation with neighboring structures and surgical planning, concomitant computed tomography was requested. Based on the clinical and radiographic findings, we chose a conservative surgical approach through marsupialization and later enucleation under local anesthesia. In the one-year postoperative period, the patient progresses satisfactorily without clinical complaints. It is concluded that the well- detailed anamnesis with a correct diagnosis, treatment and follow-up are of fundamental importance for the success of the case, decreasing the chances of recurrence and promoting quality of life for the patients.
