Reparo perirradicular em retratamento endodôntico não cirúrgico: a importância da proservação.

Abstract

Proservation endodontic treatment aims to observe the success or failure of the same, through the remission of symptomatology and the observation of the regression or repair of periradicular lesions with bone neoformation of the affected region. Thus, the present study aimed to report a clinical case with post-retreatment follow-up in a patient with extensive periradicular lesion diagnosed as chronic apical periodonditis, with an image suggestive of periapical granuloma. Teeth 21, 22 and 23 were involved, presenting also involvement of nasal fossa floor and discontinuity of the palatal bone blade. The procedure was performed by clinical, radiographic and tomographic examination before and 2 years after treatment. During the clinical examination, it was observed characteristics and normality regarding the periodontal tissues and absence of symptomatology. The area compromised by the periradicular lesion showed an expressive degree of regression and repair with signs of bone neoformation in the palatal region and floor of the nasal fossa, but without total remission until the moment. Therefore, it can be concluded that non-surgical endodontic therapy proved to be quite effective, allowing the osteolytic area to be reduced. This fact contributes to the success of the treatment. We suggest the continuation of the proservation of the case for future evaluation of the need for complementary periradicular surgery.
