The number of elderly in the world grows every day. Currently, in Brazil there are about 22 million elderly. Despite the growing number of elderly and existing programs to improve the quality of life of these people, the Unified Health System (SUS) is flawed and does not meet all the needs of this population, given that aging brings significant peculiarities and health policies should take into account the physiology of aging, bringing deficits in functional capacity of the elderly, autonomy in activities of daily living, leading to dissatisfaction. Objective: To know the reality of care offered to elderly patients with chronic lesions in the Family Health Strategy of the Municipality of Picuí – PB, and specifics: to characterize the sample beside the cultural aspects; Identify the types of chronic injuries that affect the elderly; Identify the assistance offered to elderly patients with chronic lesions; to introduce a protocol of care for chronic injuries. Methodology: This is a descriptive and exploratory research with a quantitative approach, which was developed in Services in Strategies of Primary Health Family in the urban area in the City of Picuí-PB, between January and February 2012. The sample was composed of twenty elderly of both sexes, aged equal or greater than sixty years, suffering from chronic injuries. The study was approved by the Commission of Ethics in Research of Nova Esperança Colleges. Results: The results show varied forms of chronic ulcers, highlighted by the pressure, since most were elderly, bedridden and some were wheelchair. A small representative sample showed infected ulcers with devitalized tissues and esfacelos, 75% had no granulation tissue secretion, and 25% had exudate. Most seniors had chronic degenerative disease, Hypertension and Diabetes, and conditions had impaired physical mobility, poor nutrition, and made use of immunosuppressive drugs and yet some still and some healing factors such as smoking. Dressings were implemented by professional technical and nursing assistants. In such procedures were used most prominently gases, bandages, gloves, for use as the substances have higher saline, water and soap and ointments in general. Professionals reported as guidance: home, changing position, elevation of the lower limbs (LL) with massage using oils and moisturizers. Conclusion: We believe that the study met the proposed objectives and that from the results presented it was possible to actually see the care given to the elderly by the Family Health Strategy and the need to deploy a service protocol for the treatment of chronic injuries.
