In endodontic therapy, the size of the apical enlargement appears as a controversial
issue among clinicians and researchers. Currently, the focus is in the search for
suitable size of the apical foramen enlargement and limit shutter according to the
complexity of the apical third of a tooth. This study analyzed through a literature
review the influence of foraminal expansion in endodontic treatment. We conducted a
search for scientific articles to the theme addressed clarification published between
1968-2016 and accessed in Scielo databases, LILACS, PubMed and UNICAMP
Digital Library in 2016. theses were used and national and international scientific
papers available full text relating to endodontic instrument within the foraminal
patency, diameter enlargement of the apical foramen and its extension, postoperative
pain, cleaning apical region of the periapical debris extrusion and foraminal transport
after techniques involving enlargement foraminal. According to the review carried out,
the results were favorable to the use of tactical operative enlargement of the apical
foramen. Regarding the modeling of the root apex size # 40 presents satisfactory
results in relation to enlargement and the sealing has better quality when the
instrumentation is conducted in the foramen limit. Postoperative pain wasn't observed
in vital teeth, but molars with pulp necrosis occurred slight increase in pain. The
expansion showed no interference effective in treatment. It can be concluded that the
use of foraminal enlargement presents favorable results for endodontic treatment.
