The aim of this study was to report a case of surgical treatment of gingival recession
Class III Miller. Female patient, L. L. A., 44 years old, presented with a 7mm gingival
recession with no apical keratinized mucosa in relation to the recession. The patient
reported having hypersensitivity to cold and hot, pain and ulceration of the alveolar
mucosa to tooth brushing. It was noted initially the free gingival graft surgery to
create keratinized mucosa in the apical region of the recession, aimed at preventing
the progression of the defect and foster care in the region. The gingival graft
promoted the formation of 4mm keratinized mucosa and overlaid 3.5mm of bare root
surface, corresponding to 50% of the recession. As there was the formation of a
good range of keratinized mucosa adjacent to the recession after the free gingival
graft technique and the defect was not completely covered, it was decided to move
laterally the flap in order to improve the prognosis of recession. After the technique of
laterally positioned flap, there was an increase of 1mm keratinized mucosa and
attached gingiva. However, no additional root coverage. Despite the low predictability
of the results for root coverage on a Class III Miller gingival recession, the free
gingival grafts presented to provide predictable root coverage, and especially
increased keratinized tissue, favoring the prognosis of the tooth with this recession
class.
