Abstract:
Cervical cancer (CC) is the third leading cause of death among neoplasms in women, which translates into an important public health problem, especially in less developed areas, which concentrate more than 80% of world coefficients of incidence and mortality. There are factors that contribute to improving this scenario, with the performance of the Papanicolaou exam being one of the main ones. GOALS: To analyze the knowledge, attitude and practice of the Papanicolaou test by the population women in Campina Grande-PB and its association with the prevention of CC in that population, focusing on the analysis of the factors involved in the non-adherence of these women to the preventive examination. METHODOLOGY: This is a quantitative, cross-sectional study cross-sectional, in which data were collected through forms that evaluated variables sociodemographic variables, variables related to sexual and reproductive life and adherence to the test Papanicolaou in a sample of 24 women. For statistical analysis, we used the Computational Environment R (version 3.3.2), adopting the Fischer test with level of 5% significance. RESULTS: Among the respondents, 70.8% of the patients were in age group of 50 years or more, 66.7% were from cities other than Campina Large, 62.5% had not completed elementary school, and 20.8% did not report schooling; 33.3% worked as self-employed, followed by 29.2% of retired women, and 50% of the interviewees had a family income between 1 and 2 minimum wages or least (33.3%). Of the sample, 70.8% were Catholic and 83.3% claimed to be Catholics. religion they professed. With regard to the Papanicolaou test, only 12.5% of respondents said they had no knowledge about the Papanicolaou exam before the diagnosis of Cancer of the CCU. A higher percentage, 83.3% of the patients, reported that there is a health center close to their residence, but only a range of 41.6% underwent the test as often as recommended by the Ministry of Health. Of those who did not, most cited reasons such as unaware of the need to do it (17%) and embarrassment to do it (17%) followed by the fact that they did not believe they would have the disease (15.1%). Regarding the association de outras variáveis com o estadiamento do CCU quando da detecção do mesmo, foi fator associado à frequência com que a paciente fazia o exame de Papanicolaou, sendo uma maior frequência da realização do exame, associada a um menor nível de estadiamento da doença. Fatores como idade da primeira relação sexual, número aproximado de parceiros, idade da primeira gestação, tabagismo, uso de ACO e uso de DIU foram independentes. Um maior percentual da amostra estudada (aproximadamente 70%) teve um número de gestações igual ou maior que 3 gestações. Nota-se, entretanto, que quando relacionado à variável estadiamento do CCU em sua detecção, as duas maiores frequências estão entre mulheres que tiveram duas gestações (Estadiamento IB1/T1b1) (66,7%), seguido por ou 5 ou mais gestações (Estadiamento IIIB/t3b) (55,6%). Observa-se assim, que mesmo sendo detectado o CCU com uma maior frequência entre mulheres com duas gestações, o estadiamento do mesmo é mais grave quando detectado entre mulheres com número de gestações maior ou igual a 5. CONCLUSÃO: Fatores como baixo nível de conhecimento e a maior quantidade de gestações, assim como a não adesão ao exame preventivo com a frequência recomendada pelo Ministério da Saúde parecem estar relacionados tanto com a maior prevalência quanto com o estádio mais grave de detecção do câncer de colo uterino na população estudada. A taxa de adesão ao teste de Papanicolaou se deu em um percentual menor do que o observado em outros estudos populacionais brasileiros, tendo a maioria das mulheres apontado, como motivo de não adesão ao exame preventivo, justificativas de cunho muito mais individual do que organizacional/pragmático. Devido ao número amostral haver sido pequeno, faz-se necessária a realização de outros estudos, para a consolidação dos resultados encontrados.
