Medication reconciliation in hospitals is essential to ensure the review of pharmacotherapy and patient safety. However, it faces challenges such as communication failures among healthcare teams, leading to discrepancies between medical prescriptions that can pose a risk to the patient. Therefore, the objective of this study was to analyze the medication reconciliations performed in a university hospital and to outline the main related indicators. This is a descriptive study of the medication reconciliations performed in the female and male medical clinic wards from December 2023 to May 2024 at the Alcides Carneiro University Hospital. The two wards analyzed a total of 251 medication reconciliation records, of which 68 were in accordance with the medical prescription. However, 161 medication discrepancies were identified in the reconciliation forms, 140 of which were intentional and 21 were unintentional. In total, 38 pharmaceutical interventions were performed, which resulted in the alteration of 19 medical prescriptions. Based on the research carried out, the occurrences of the main medication discrepancies were identified, with the medical decision not to prescribe being the most frequent intentional discrepancy and omission being the most repeated unintentional discrepancy. From the data analyzed, it is possible to see how pharmaceutical interventions can be an important ally in reducing such events and ensuring patient safety.
