The transition period between childhood and life adult corresponds to adolescence, characterized by the impulses of development, physical, mental, emotional, sexual and social, and also by personal attempts to target the objectives that correspond to the cultural expectations of the society in which lives. The increasingly discovery early of sexuality is a reality notorious among adolescents, requiring more attentive assistance from health professionals, especially of nurse, OBJECTIVES investigate the dynamics of nursing actions in the ESF intrinsic on sexual and reproductive health of adolescents, approach on family planning; Analyze how the consultation nursing of adolescents in the UBS; Is performed the methods of approach of the nurse METHODOLOGY: This study is an descriptive exploratory research with qualitative approach in the city of Picui-PB. The data was analyzed and used through Thematic Content Analysis technique. The study was initiated after examination and approval by the Ethics Committee, whose protocol number of the Certificate of Ethics Presentation was 71645317.3.0000.5182. RESULTS AND DISCUSSIONS: It was evidenced that the majority of nursing professionals are young and female of sex. The knowledge of the professionals is fundamental in the development of the actions carried out; ability and understanding to provide correct information regarding family planning. It was found that it's still a reality present in the health services due to the lack of actions aimed at the target public of the study, principally on sexual and reproductive health, it's noticed in the speech of the professionals that there are many difficulties in the implantation of an assistance that is efficient in promoting and preventing adolescent health problems, but recognizing that there is a need to work toward this challenge is an important step. Unfortunately the services still present weaknesses that need correction, but that depend not only on the professionals involved in the care but also on the managers who should pay attention to the lack of resources and inputs to able of attracting, not limiting the demand for the service only as a responsibility of the adolescent. CONCLUSION: The assignments of the nurse in the FHT are broad, allowing the professional to explore a wide field in the aspect of assistance: in the promotion and prevention of problems, treatment and rehabilitation of the population health. There are gaps in the assistance provided, ranging from lack of capacity to act on the issue with the public, lack of specific groups for working with adolescents. the limits of the research that give margin to carry out new studies on the subject, some of them: the research was carried out in only one municipality; restricted in Basic Health Units of the urban zone, excluding the professionals working in the rural area, due to the difficulty of geographical location and access of the researchers to the possible participants.
