Résumé:
The increase in life expectancy and the aging process mean that the elderly are more vulnerable to the development of some diseases, among them tuberculosis, but the lack of knowledge of the managers about tuberculosis control actions and their attributions for triggering these diseases. Actions, lead to delayed diagnosis. The objective of this study was to analyze the management subjects' discourse about the organization of health services and the practice of planning actions for the control of tuberculosis related to the elderly. A qualitative study was carried out in nine municipalities belonging to the Curimataú/PB region. Nine members of management teams participated in the study. Data collection was performed from February to May 2016, through a semi-structured interview. To analyze the data, we used the theoretical-methodological device of Discourse Analysis, of French matrix. The managers' discourses evidenced the lack of planning for actions to control tuberculosis in the elderly, the concern of managers turning to tuberculosis only in times of campaigns and D-days. The tuberculosis control actions are performed only through the content addressed in Basic Care notebooks and protocol. The managers reveal in their speeches that actions such as active search, assessment of communicators and follow-up of the cases are actions carried out in health units. Tuberculosis is placed in the background, it is not a disease that requires attention to planning control actions, the emphasis for action planning is given to diseases that are in greater evidence, although they recognize that tuberculosis is increasing in the elderly population. Although the elderly have a senescent immune system and atypical symptomatology, the planning actions do not include the elderly population, since they are not considered a risk group for tuberculosis. The managers' discourses are linked to a hegemonic management model and a biomedical care model. Managers do not position themselves as one of those responsible for planning, when planning is done is focused solely on disease. It is evident that there is a need to reorganize the health service and that health managers and professionals go through qualification processes, so that they can understand the importance of planning, and also reflect on the work process, with the aim of better Assist the elderly population.
