Abstract:
Public Mental Health in Brazil has been modified and has been gradually built to meet the needs of professionals, users of the system and their families. Mental health has a broad dimension, which is constituted by several complex phenomena that are in constant transformation. In this proposal for the reconstruction of psychiatric care, the Psychosocial Care Centers (CAPS), regulated according to the ordinance of no. 336, dated February 19, 2002. They constitute strategic services, replacing the asylum model. The objective of this study is to prepare a tool for follow-up of mentally ill patients in psychiatric nursing care, to be used by academics and professionals, in order to facilitate the care provided at the Psychosocial Care Centers (CAPS). emphasizing the data collection for the accomplishment of the Systematization of Nursing Assistance - SAE. It is an action research of bibliographical character and qualitative approach. The construction of the instrument was carried out from a pre-analysis in the contextualization of the content addressed, through readings in books, articles, among other bibliographic collections, besides the identification of some data relevant to the process of an integral service, such as the data sociodemographic data, informational data and psychic evaluation. The research was carried out in the CAPS of the city of Cajazeiras, based on the activities and interventions developed from an extension project entitled Mental Health: The Psychiatric Nursing Consultation implanted in the CAPS. The elaboration of the follow-up instrument for mentally ill patients constitutes a proposal to improve the care of these patients in the field of psychiatric nursing. It is recommended that the instrument be applied in Psychosocial Care Centers of all levels and modalities, with the purpose of attending patients in a holistic and integral way, guaranteeing humanization in the process and the effectiveness of nursing care.
