A tuberculose é uma doença infecciosa, causada pela bactéria Mycobacterium
tuberculosis. Embora sejam realizadas estratégias para o controle dessa doença, ainda é
elevada a sua incidência e mortalidade, sendo apresentada como um problema de saúde
pública. Mesmo que venham ocorrendo mudanças no cuidado ao doente de tuberculose,
voltadas para a atenção básica, ainda há fragilidades que contribuem para a não adesão ao
tratamento. O que reflete na persistência de cepas multirresistentes, bem como na
continuidade da morbidade e mortalidade, repercutindo de forma negativa no controle da
doença. Contudo os princípios básicos para a realização do tratamento referem-se a
associação medicamentosa adequada e utilização das doses corretas por tempo suficiente.
Evitando assim a persistência bacteriana e a resistência farmacológica, assegurando a cura do
paciente. Objetivo: Apresentar, por meio da literatura, como é realizado o Tratamento
Diretamente Observado da tuberculose na Atenção Primária à Saúde. Metodologia: Trata-se
de uma revisão integrativa da literatura, a qual possibilita a síntese do conhecimento e a
incorporação da utilidade de resultados de estudos significativos na prática. A pesquisa foi
realizada nas bases de dados SCIELO e BIREME, onde foram utilizados como critérios de
inclusão para seleção: Artigos científicos completos, publicados em português, nos últimos
cinco anos (2012-2016), que estivessem disponíveis na íntegra e que abordassem a temática
escolhida para o presente estudo. Foram utilizados os descritores “tuberculose”, “tratamento”,
“enfermagem” e “Atenção Primária à Saúde” Resultados e Discussão: Foram encontradas
477 publicações, nos quais após a utilização dos critérios foram identificados 12 artigos para
avaliação criteriosa. Da análise emergiram cinco categorias: abordando o conhecimento dos
profissionais acerca da tuberculose e o Tratamento Diretamente Observado; os desafios para a
realização do Tratamento Diretamente Observado; a equipe multiprofissional no tratamento
da tuberculose; a necessidade de educação continuada direcionada para a assistência ao
portador de tuberculose; e por fim, referiu-se as recomendações para a prática dos
profissionais no Tratamento Diretamente Observado da tuberculose. Foi possível observar
fragilidades no atendimento integral, mostrando a importância de formação de uma equipe
multiprofissional no cuidado ao doente de tuberculose, foi também evidenciado a presença de
fatores relacionados tanto ao paciente quanto aos profissionais que contribuem para a não
adesão ao tratamento, além de um déficit de conhecimento dos profissionais em relação a
doença, considerando a relevância de educação continuada e capacitações para a equipe da
Atenção Primária à Saúde , a fim de promover melhoras nas ações educativas e assistenciais.
Considerações Finais: O estudo demonstrou que há necessidade dos profissionais de saúde
em geral e enfermeiros da Atenção Primária à Saúde, desenvolverem um melhor preparo para
lidar com o paciente de tuberculose, estando capacitados para oferecer orientações, cuidado
integral, atendimento e supervisão adequada do tratamento, envolvendo uma equipe
multidisciplinar, visando à qualidade da assistência e à promoção da saúde. |

Tuberculosis is an infectious disease caused by the bacteria
Mycobacterium tuberculosis. Although there are strategies to control it, its incidence and
mortality are still high, being presented as a public health problem. Even with important
changes at the primary health care in the assistance to tuberculosis (TB) patients, there are
still fragilities that lead to non-adherence to treatment, what collaborate to persistence of
multidrug-resistant strains. Therefore, its morbidity and mortality are mantained, and this
impairs the disease control. With regards to the treatment, it consists in the use of an
appropriate combination of drugs in correct doses and for long enough. Thus, avoiding the
bacterial persistence and drug resistance, so ensuring the patient`s cure. Objective: To
present, through the pertinent literature, the way that the Directly Observed Treatment of
tuberculosis is done at the Primary Health Care. Methodology: This is an integrative
literature review, which enables the synthesis of knowledge and the insertion of the
significant results of the main studies, to the practical conducts. For the survey, the SCIELO
and BIREME databases were used, and the inclusion criteria for selection were: complete
scientific articles published in portuguese, at the last five years (2012-2016), available in full,
and pertinent to the topic of this study. The descriptors: "tuberculosis", "treatment", "nursing"
and "Primary Health Care" were used. Results and Discussion: 477 publications were found,
of which, after using the points of criteria, we identified 12 articles for a detailed evaluation.
From the analysis, five categories emerged: the knowledge of the health professionals about
tuberculosis and the Directly Observed Treatment; challenges to the realization of the Directly
Observed Treatment; the multidisciplinary team to treat tuberculosis; the need for continuous
education directed to the care of TB patients; and the recommendations for the professional
practices on the Directly Observed Treatment of tuberculosis. We observed fragilities on the
integral attendance, revealing the need to forming a multidisciplinary team for the care of TB
patients; was also evidenced the presence of factors related to both, patient and professional,
that lead to non-adherence to treatment; and the professionals demonstrated a lack of
knowledge about the disease, pointing to the importance of the continuous education, and
training for the team of the Primary Health Care, in order to improve the activities of
education and health care. Final Considerations: The study evidenced that the professionals
working at the basic health attention, including nurses, need more instruction to deal with TB
patients, being able to offer guidance, integral care, assistance and adequate supervision of the
treatment, involving a multidisciplinary team, aiming at the quality of care and the health
promotion. |
