Topiramate is a drug derived from D - fructose. Initially used for their hypoglycemic action and different from other antiepileptic drugs chemical structure. Has a mechanism of action with plexus has recognized clinical efficacy in epilepsy and migraine. This drug has multiple mechanisms of action, such as blocking sodium , potentiation of GABA inhibition , reduced ativity excitatory glutamate , inhibition of calcium channels and high voltage anhydrase inhibition Carbonica channels. The clinical innovations, new studies and pharmacological evidence for other diseases such as epilepsy, alcoholism and addiction , glaucoma and myopia, compulsive eating and obesity, makes this drug a target for further studies in order to obtain a better efficacy and the same therapeutic indication. This literature review aimed to glean important information about this drug, seeking research data in the Virtual Health Library (VHL), indexed in LILACS and SciELO, Bireme ScholarGoogle and CAPES Portal database. Several studies that confirmed the action of topiramate, which is used for inclusion and exclusion criteria for this study were found. Review articles and clinical trials have shown that topiramate is a drug that acts on epilepsy, obesity and alcoholism. Therefore, topiramate is an innovative drug last generation that still needs a lot of study to understand the whole mechanism on various health conditions.
