Resumo:
The significant increase in the incidence and consequence of the prevalence, morbidity and
mortality and high costs caused by hypertension and diabetes mellitus, makes us prioritize the
various lines and HIPERDIA care program offered by the family health strategy. This study
aimed to measure the level of attention focused on the quality offered in the health system
cajazeirense, with of the application Assessment of Chronic Illness Care – ACIC version 3.5,
identifying vulnerabilities and developed the program, allowing the development of a plan of
interventions consistent and objective, compared to chronic diseases not transferable. This is a
survey of exploratory and descriptive nature, field, referring to the quantitative approach with
HIPERDIA held in the city of Cajazeiras-PB between the months of April to October 2012.
From the application of the ACIC, didactically divided, to better meet the needs of the
research in three focal areas: Policies and Resources, responded by primary care manager of
the municipality by the Community responded by president municipal health council and the
Health Unit, accounted for 12 units of family health that met our inclusion and exclusion
criteria, a total of 14 research subjects. The collected data were transcribed and analyzed
using descriptive statistics with the help of software Microsoft Office Excel 2010 and
Microsoft Office Word 2010. The study subjects were mostly female (85.7%) aged 30 to 35
years (35.7%), with higher education in nursing (92.9%) and represented in the application, in
Most of the family health unit (85.72%). Obtained as a result considered expected, normal,

assessment permanently.
